The Hospice Social Worker provides direct casework and counseling services to the patient/family unit. The Social Worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.

We offer nursing, aide, social, chaplain, bereavement, and volunteer services along with medical equipment, supplies, and medications (related to the diagnosis) for people in the last phase of an incurable disease so that our patients live as fully and comfortably as possible.
